This Digital Affidavit is executed electronically on 08 / 03 / 2026 by the undersigned Volunteer through the official website of the Organization.

2. Declaration

I, the above-named deponent, do hereby solemnly affirm and declare that:

I have voluntarily applied as a Volunteer under the “Project Youth” programme implemented by Minal Yuva Sangathan.
All personal, educational, identity and bank details provided by me are true, correct and complete.
If any information is found false or misleading at any stage, the Organization may cancel my registration without any liability.
I am presently unemployed and fulfill the eligibility criteria of the programme.
I voluntarily undertake to complete 20 survey forms per working day, categorized as:
- 05 – Rural Education
- 05 – Rural Health Care
- 05 – Rural Self-Employment
- 05 – Youth Self-Employment
I agree to complete the prescribed six (6) months Training and Survey period.
In case of failure to complete assigned targets or non-compliance with instructions, I shall not be entitled to any Honorarium or remuneration.
I agree to follow all rules, guidelines and directions issued by the Organization.
In case of negligence, misconduct, or submission of false data, the Organization may take disciplinary or legal action, which shall be binding upon me.
I shall not raise any civil, criminal, labour, financial or compensation claim against the Organization or its office bearers.

3. Electronic Execution Clause

This Affidavit is executed digitally through the official website. By clicking “I Agree” and submitting this form with OTP verification, I provide legally valid electronic consent under applicable Indian laws.

CONTRACT (Project Youth Plus)

This Online Volunteer Service Contract (“Contract”) is executed electronically on 08 / 03 / 2026 by and between:

(Hereinafter referred to as the “Organization”)

SECOND PARTY

The individual registering through the official website of Minal Yuva Sangathan
(Hereinafter referred to as the “Volunteer”)

By completing the online registration form and clicking “I Agree”, the Volunteer accepts the following legally binding terms:

1. Nature of Engagement: 1.1 The Volunteer is engaged purely on a voluntary and performance-based basis. 1.2 This Contract does not create any employer-employee relationship. 1.3 The Volunteer is not entitled to statutory employment benefits.

2. Scope of Work: The Volunteer shall conduct survey work in the following sectors as assigned: Health, Education, Rural Self-Employment, Youth Self-Employment.

3. Work Structure & Targets: 3.1 26 working days per month. 3.2 20 survey forms per working day (5 in each category). 3.3 Participation in 4 Sundays per month in social activities is mandatory.

4. Performance-Based Honorarium: 4.1 Upon successful completion of 26 × 20 verified forms: ₹10,000/- Honorarium, ₹2,000/- Travel Allowance, Total ₹12,000/- per month. 4.2 Payment shall be released only after verification of submitted survey forms. 4.3 Failure to meet target may result in zero payment. 4.4 The Organization reserves the right to terminate the engagement without liability.

5. Registration Fee: 5.1 A voluntary registration fee of ₹300/- (kit & processing) is non-refundable. 5.2 Payment of registration fee does not guarantee honorarium.

6. Data Authenticity: 6.1 The Volunteer shall submit only genuine and verified survey data. 6.2 False, manipulated, or duplicate entries shall result in immediate termination and possible legal action.

7. Termination Clause: The Organization reserves absolute right to suspend or terminate the Volunteer at any time without prior notice or reason.

8. No Claim Declaration: The Volunteer agrees that no civil, criminal, labour, financial or compensation claim shall lie against the Organization, its President, trustees, or office bearers.

9. Electronic Acceptance: 9.1 This Contract is legally valid under the Information Technology Act, 2000 (India). 9.2 By clicking “I Agree” and submitting the form, the Volunteer provides legally binding electronic consent. 9.3 Typed name and OTP verification shall constitute valid digital signature.

10. Jurisdiction: All disputes shall be subject to exclusive jurisdiction of courts at Saket, New Delhi.
